Roster- your Official Hockey Saskatchewan Team Roster must accompany this registration form.
      (You may have to obtain this from your local minor hockey association)
-The Official Hockey Saskatchewan Team Roster must include all players last name, first name, sex,
DOB and position as well as team officials.
-You may hand write in the jersey numbers on the Official Hockey Saskatchewan Team Roster or
            provide a separate list. Sorry no Provincial rosters will be accepted.
-Registered AP names with birth dates and jerseys numbers will be accepted up to
1 week prior to the commencing of the tournament.
-It is your teams’ responsibility to provide a new Official Hockey Saskatchewan Team Roster shall there
          be any changeschanges will be accepted up to 1 week prior to the commencing
of the tournament.
-Only those players listed on your Official Hockey SaskatchewanTeam Roster and your registered APs
list will be permitted to play in the tournament.
